Step 1: Read Terms and Conditions of Service

If you would prefer to mail or fax in a letter of authorization, we've made that letter available for download. Simply print it out, fill it in; sign it, then remit it to us. You will need Adobe Acrobat Reader to view this document. If you don't have it, you can get it from Adobe's web site.
